What Should You Have Ready for a Rehab Insurance Verification Call?
Have your insurance card, member details, and information about the proposed outpatient treatment nearby. Use the call to ask about benefits for the exact services and provider—not simply whether “rehab is covered.” Keep preliminary answers separate from confirmed costs and approvals.
Gather the information for your call
Use this preparation list, then ask the person handling the benefits check which details they need:
- Insurance information: Your card, including the insurer’s name, member ID, group number if listed, and member services number.
- Member details: The patient’s name and date of birth, plus policyholder information if different.
- Proposed treatment: Whether you are discussing standard outpatient alcohol treatment or intensive outpatient treatment, with any service descriptions or billing codes supplied by the provider.
- Provider information: The treatment location and billing provider details for the network inquiry.
- Other insurance: Details of another health plan, if applicable.
If you are helping an adult family member, ask what permission is needed to discuss their benefits. Share identifying information only through a verified contact or secure process, never in public comments or unsolicited messages.

Ask about your potential financial responsibility
Request answers for the proposed services, provider, and location:
- Deductible: Does one apply, how much remains, and which proposed charges would count toward it?
- Copays and coinsurance: What fixed amounts or percentage-based charges would you owe for each service?
- Network status: Is this provider and location in-network under your specific plan? If not, are out-of-network benefits available, and what additional charges should you ask the provider about?
- Conditions: Are there relevant exclusions, service limits, referral requirements, or authorization steps?

Separate benefits, authorization, and payment
Do not treat a benefits check as authorization or guaranteed payment. Ask explicitly: “Have you only checked benefits, or has any required treatment authorization also been obtained? What conditions could still affect payment or my final bill?”
Record the call date, representative’s name, any reference number, and unresolved questions. Ask who will handle each remaining step. Request a written provider estimate, including separate charges, and clarify which amounts remain estimates rather than confirmed obligations.

To ask Blue Coast about available treatment and insurance verification, call 949-776-2127: “Which outpatient services are available, what information do you need to check my benefits for the services being considered, and what will still need confirmation?”
